REGISTER AN AED OWNED BY YOU THAT IS LOCATED AT YOUR HOME ADDRESS

As part of the process of applying to become a member, you will be required to provide proof of, or agree to the following criteria:

1. Proof that you reside at the address, or are the tenant/landlord at the address you provide us with as the location of the AED;

2. Acknowledge that you provide consent for Heart of the Nation to use your address details provided and accessibility times of the AED as part of the Heart of the Nation Network;

3. Commit to displaying the Heart of the Nation sticker that you will be provided with at all locations with AED's.

4. During the hours that your AED is accessible, allow your AED to be used in the event of a member of the public requiring access to life saving defibrillation.

5. Provide consent for us to pass your information on to Ambulance Services in your State or Territory to form part of a register to allow Triple Zero call takers to guide bystander-responders to the nearest AED.

What You Will Need to Register:

To fill in this application fully, you will need:

Evidence of residency at the address you provide us with, or that you are the owner/landlord or tenant at that address - eg. rates notice, water bill, etc;
Photo of the AED in the location where it is usually kept.
Latitude and longitude data (GPS coordinates) of the location of the AED - you can either enter these manually, or using your current position if you are standing next to the AED - please ensure that you have location services turned on in your phone's settings for your internet browser. Manually, to get the GPS coordinates, stand next to the AED and get these from Google Maps or What3Words by clicking on the "set my location" icon - usually an arrow.
Hours of availability of the AED - is it only when you are home, or is the AED located externally, and is accessible at any time?

This will be the name of the location of the AED that will be displayed on mapping of the AED so that people can find it easily. It should correlate to the name of the business or organisation that is displayed, or other another known name of that location, eg. Smalltown Airport

b) Address of where AED is located - this should be the address that someone would drive to if they were to access the AED*

c) Describe where the AED is at the location

Next, we'd love you to enter the GPS coordinates of the AED to help provide pin-point accuracy for those who need to find it. This is not the GPS co-ordinates of the "address" of the AED, but the GPS co-ordinates of where it is actually located. Please go to your settings in your phone or your internet browser on your laptop or desktop, and ensure that location services are allowed for your internet browser you are using to complete this form, then re-open this window. When you press the "Auto-fill Location" button below, you will be asked to grant permission, or deny permission. If you grant permission, your location will be auto-filled, so please ensure you are standing next to the AED when pressing this button and granting permission. I consent to Heart of the Nation using our location data for an AED locator app to allow members of the public to find our AED in the event of a cardiac arrest, where it is one of the nearest AED's to their location.

I consent to Heart of the Nation passing on our location data to the appropriate State ambulance service for those states that have an AED register operated by State ambulance services to allow Triple Zero call operators to guide members of the public to find their nearest AED in the event of a cardiac arrest.
